Kaiser Wellness Referral

The Kaiser Wellness team provides social emotional, behavioral, academic, and physical health support to students, teachers, and families with an emphasis of on proactive supports. Members of the wellness center team include the Principal, School Psychologist, SAIL District Specialist, School Social Worker, School Nurse, Community Facilitator, and other District Specialists.

Please complete this form to identify student(s) for whom you have social-emotional, academic, behavioral, or health concerns. Upon receipt of this form, the Kaiser Wellness team will review your concerns during scheduled meetings, occurring monthly, and contact you regarding next steps for support.
